Therapeutic Penetrating Keratoplasty Surgery In Patiala

Therapeutic Penetrating Keratoplasty Explained

Therapeutic Penetrating Keratoplasty (TPK) is a surgery where a damaged or infected cornea is replaced with a healthy donor cornea. This is done to treat serious eye infections or damage that can’t be fixed with other treatments.

Here’s how it works:

  • Purpose: TPK is used for treating severe eye infections like bacterial, fungal, or viral infections that haven’t responded to other treatments. It can also repair the cornea if it has a serious hole or tear.

  • Procedure: The surgeon removes the damaged part of the cornea and replaces it with a donor cornea that is slightly larger than the area being treated. The new cornea is stitched in place with very fine stitches.

  • Additional Steps: The surgery might include removing other parts of the eye if needed, and antibiotics are used to prevent new infections.

  • Recovery: After the surgery, antibiotics and sometimes steroids are used to help the new cornea heal and to prevent further infection.

TPK is a common and important procedure for treating severe corneal problems, especially in countries with limited resources.

Types of Therapeutic Keratoplasty

  1. Full-Thickness Therapeutic Keratoplasty: This involves replacing the entire thickness of the cornea with a donor cornea. It is used to treat serious infections or damage that affects the whole cornea.
  2. Tectonic Keratoplasty: This type focuses on repairing the structural integrity of the cornea, especially if there are large perforations or severe thinning. It helps to restore the cornea’s shape and function.
  3. Patch Graft: A smaller piece of donor cornea is used to cover a damaged area. It is a less extensive procedure compared to full-thickness keratoplasty and is often used for specific areas of damage or infection.
  4. Therapeutic Deep Anterior Lamellar Keratoplasty (DALK): Instead of replacing the entire cornea, this method removes and replaces only the front layers of the cornea. It is used for conditions affecting just the outer layers, leaving the deeper layers intact.
What is the role of preoperative antimicrobial therapy in TPK?

It reduces the risk of infection before the surgery.

What follow-up care is required after TPK?

Patients need regular check-ups to monitor healing and prevent complications.

What are the complications of DMEK surgery?
DMEK carries a risk of postoperative IOP elevation due to pupil blockage caused by the air or gas bubble in the AC, or angle closure due to anterior iris displacement caused by bubble migration to the posterior chamber. A postoperative IOP rise occurs most frequently within the first two hours following DMEK.
Can TPK restore vision?

Yes, TPK aims to restore vision by replacing the damaged cornea.

What are the benefits of using the latest technology in TPK?

Advanced technology improves precision, reduces risks, and speeds up recovery.

What is the success rate of TPK?

Success rates are high, but vary depending on the condition being treated.

How is the donor cornea prepared for TPK?

The donor cornea is carefully matched in size and shape to fit the recipient’s eye.

What is the recovery time for TPK?

Recovery can take several weeks to months, with regular follow-up visits.

Are there risks with TPK?

Risks include infection, graft rejection, and complications from surgery.

How long does TPK surgery take?

The procedure usually lasts a few hours, depending on the complexity.

How does TPK differ from other keratoplasties?

TPK replaces the entire cornea, while other types may only replace certain layers.

Technology Available for Therapeutic Penetrating Keratoplasty at LJ Eye Institute – Patiala

At LJ Eye Institute – Patiala, we utilize the latest technology to ensure effective and precise therapeutic penetrating keratoplasty (TPK). Our advanced tools and techniques include:

  1. Femtosecond Laser: This laser provides highly accurate cuts, allowing us to precisely remove the damaged cornea and prepare the donor tissue. It enhances the accuracy of the procedure and improves healing.
  2. Optical Coherence Tomography (OCT): OCT gives detailed, cross-sectional images of the cornea, helping us assess its condition and plan the surgery. It aids in monitoring the cornea before, during, and after the procedure.
  3. Specular Microscopy: This tool measures the health and density of the corneal endothelial cells, essential for evaluating the need for TPK and checking the success of the transplant.
  4. Corneal Topography: This technology maps the surface of the cornea to provide information about its shape and thickness. It helps customize the TPK procedure to each patient’s specific needs.
  5. Digital Imaging Systems: These systems provide high-resolution images for precise surgical planning and monitoring of the graft integration and healing process.

By incorporating these advanced technologies, LJ Eye Institute – Patiala ensures that therapeutic penetrating keratoplasty is performed with the highest level of precision and care, leading to better outcomes and faster recovery for our patients.
