Optical Penetrating Keratoplasty Explained
Corneal diseases can lead to blindness and vision problems, especially in developing countries. Corneal transplantation, or corneal grafting, is a surgery where a damaged cornea is replaced with a healthy one from a donor who has recently passed away and was free from diseases that could affect the tissue.
While newer methods exist, penetrating keratoplasty (PK) is still considered the best option for serious corneal issues that involve the entire thickness of the cornea. There are two main types of corneal transplants:
- Penetrating Keratoplasty (PK): This involves replacing the entire cornea with tissue from a donor eye.
- Lamellar Keratoplasty (LK): This involves placing a partial thickness graft in the eye after removing only the damaged front part of the cornea.
Penetrating keratoplasty remains a key treatment for severe corneal conditions.
Why is corneal transplantation needed?
It is needed when the cornea is damaged or diseased, affecting vision and causing blindness.
What types of corneal transplantation are there?
The two main types are penetrating keratoplasty (PK) and lamellar keratoplasty (LK).
What are the indications for PK?
The specific indications for PK are varied and can include corneal dystrophies and degenerations, congenital opacities, bullous keratopathy (BK), trauma, keratitis, and immune rejection and/or failure of a previous corneal transplant
What is penetrating keratoplasty (PK)?
PK involves replacing the entire cornea with a donor’s corneal tissue.
What is the vision after penetrating keratoplasty?
What is lamellar keratoplasty (LK)?
LK involves replacing only a part of the cornea, using a donor graft.
What makes penetrating keratoplasty different from lamellar keratoplasty?
PK replaces the whole cornea, while LK replaces just part of it.
How does corneal transplantation help with vision problems?
It replaces damaged corneal tissue, restoring vision and improving clarity.
What is lamellar keratoplasty (LK)?
LK involves replacing only a part of the cornea, using a donor graft.

Tectonic grafting may be carried out to restore or preserve corneal integrity in eyes with severe structural changes such as stromal thinning and descemetocele.
Therapeutic corneal transplantation may afford removal of infected corneal tissue in eyes unresponsive to antimicrobial therapy.
Cosmetic grafting may rarely be performed to improve the appearance of eye.
