What is DMEK surgery and why it is needed?

Descemet’s Membrane Endothelial Keratoplasty (DMEK) is a type of eye surgery that replaces just the inner layer of the cornea. This procedure is used when the cornea’s innermost layer, called the endothelium, becomes cloudy due to disease or injury.

How DMEK Works:

  • Selective Removal: The surgery removes only the Descemet’s membrane and the endothelium from the cornea.
  • Donor Tissue: These layers are replaced with donor tissue that is very thin, about 10-15 microns thick (thinner than a human hair).

What are the restrictions after DMEK surgery?
DMEK, DSAEK, and PK patients should avoid heavy lifting and bending past the waist for the first week after surgery. Patients receiving a PK must protect their eye for the rest of their life and avoid any kind of full-contact activities that could result in a direct blow to the eye.
Can I drive after DMEK surgery?

 You should avoid driving until your doctor gives you the go-ahead, usually once your vision has stabilized.

What are the complications of DMEK surgery?
DMEK carries a risk of postoperative IOP elevation due to pupil blockage caused by the air or gas bubble in the AC, or angle closure due to anterior iris displacement caused by bubble migration to the posterior chamber. A postoperative IOP rise occurs most frequently within the first two hours following DMEK.

Expect some discomfort and blurred vision initially, but these usually improve as the cornea heals.
